Health Guarantee: 1 year
WE DO NOT GIVE REFUNDS OR PAY FOR VETERINARY BILLS UNDER ANY CIRCUMSTANCES

One additional year will be added after proof of spay/neuter has been received before said dog has reached 10 months of age.

PUPPIES whose age at a date of sale precludes completion of the scheduled series of inoculations and or worming medications, it is necessary for the purchaser to complete the scheduled series of inoculations and or worming medications ON TIME, in order to protect the puppy and for this health guarantee to remain valid.

A timetable for this series of inoculations will be supplied on the medical history of the puppy. In
addition, until this series of inoculations has been complete (about 16 weeks of age) the puppy should not be exposed to premises known to be suspected of being contaminated or unhealthy animals. (Rest stops, pet parks, Pet shops where
multiple animals with a variety of contagious viruses can be easily transmitted,
other dogs from friends, etc,)

Buyer has researched the breed and is aware of any common breed related illnesses and conditions.

Buyer has researched and is knowledgeable of the non-life threatening problems that a dog may have, such as coccidia, giardia, parasites and kennel cough, Parvo, Distemper etc. as those can be contracted at any time the puppy leaves our home as it can be transmitted easily, and agrees that these are not covered under this health
guarantee.

Initial ________________

Sometimes there is an occasional umbilical hernia on puppies that will usually resolve during their adolescence. If it does not it can be taken
care of at time of spay or neuter
(Doodles and More also not responsible for this procedure).

The buyer is advised to have this puppy examined by a competent veterinarian
of the buyers' choice within 2 business days to assure that they have purchased a healthy puppy, free from major life threatening health problems. This will be done at the purchaser's expense.

Under no circumstances will Carrie Firestone Or anyone associated with Doodles and More be responsible for any
veterinarian bills acquired by the purchaser for this puppy.

initial:_______

Minor, breed related, treatable or correctable conditions (such as Patellar
Luxation, Legg-Calvé-Perthes Disease, PRA-Prcd, Umbilical hernias, or kennel cough, ear infections, yeast infections, parasites, etc), or any condition that may go away or is considered to be a condition which a puppy will likely outgrow, easily treated or any cosmetic non-life threatening related issues (such as and not limited to, misaligned bite/ dental issues) are not covered.

WE DO NOT GIVE REFUNDS OR PAY FOR VETERINARY BILLS UNDER ANY CIRCUMSTANCES

We will give a replacement puppy for life threatening genetic diseases only.

initial:_______

If a health issue arises within the first TWELVE MONTHS of age which your veterinarian believes could possibly be a life threatening genetic disease the following steps need to be taken:

(1) A written explanation from a licensed veterinarian and backed up with lab work or x-rays as to the nature of the problem.

(2) A second opinion from a veterinarian of breeders choice to examine the puppy for signs of abuse, conditions due to environment or for life threatening congenital defect.

(3) Proof that the puppy was examined by a certified veterinarian no later than 2 working days after receiving the puppy.

(4) Show proof of spay or neuter if said puppy is old enough to undergo procedure.

If the Vet's prognosis is that the puppy has a life threatening genetic disease and all of the above criteria are met, we will replace a puppy of equal value when one becomes available.

No problems other than life threatening
congenital defects are covered in this agreement.

We offer this guarantee until the puppy reaches one year of age. If at any time you transfer ownership of the puppy, this agreement is void immediately.

We are not responsible for the heath of this puppy once buyer has taken physical owner ship of puppy. Except for life threatening
congenital defects.

This is due to the possibility that the dog may become infected by a communicable disease, such as kennel cough, Parvo, etc, after its delivery to
the new owner. If the puppy is not examined within 48 business hours after buyer takes possession the health guarantee is null and void and the buyer
assumes full responsibility for the puppy’s condition.

BUYER'S RESPONSIBILITY:
Have the puppy examined by a licensed veterinarian within 48 working hours of delivery.
If a problem does arise, the Breeder is to be notified immediately following the examination of the puppy. The problem must also be verified by the Breeder's own veterinarian.

Provide the puppy with routine preventive care, such as, but not limited to the following:
Annual physical examinations by a licensed veterinarian.
Follow veterinarian guidelines for vaccinations and de-worming.
